Group IAP 2019/2020 Call for Proposal Submission Checklist

Please use the below checklist to ensure when submitting your Proposal you have included all required information.

  • Completed Group IAP Application Form (signed by legal signing authorities)
  • Original Group IAP Consent Forms (completed and signed) *unless already submitted
  • Proof of Incorporation for non for profit organization
  • Financial Statements (if required)

Once the application is complete please ensure you mail the package to:
